難治性潰瘍性大腸炎の特徴像―内科の立場から

要旨　クロストリジウムディフィシル（CD）腸炎やサイトメガロウイルス（CMV）腸炎は潰瘍性大腸炎（UC）の経過中に発症する感染症として一般的であり，特にUCに合併したCD腸炎，CMV腸炎での手術率や死亡率は一般集団に比べ増加傾向にある．両者の発症危険因子は類似しており，宿主の免疫低下状態との関連が深い．両者の診断に難渋する症例も多く，複数の診断手法を用い精度を高めることが重要である．また，難治性UCにおける上記腸炎は併発することも多いと考えられ，常に両者の混合感染に注意を払うことが大事である．
