―消化管感染症の病理―形態診断に役立つ病理組織所見

はじめに

　消化管は口腔から始まり肛門まで外界と交通している．大腸内には常在細菌叢が形成されている．呼吸器系と同様に，無菌野ではないため，培養のみが施行され病原体が検出されたとしても，“汚染菌”か“真の感染原因菌”を培養検査結果のみから鑑別することは困難である．病理組織診断法は，感染症病原体の同定に加えて，既存組織の出血，壊死，潰瘍形成の有無などから“真の感染症”が診断可能である．本稿では，消化管感染症の形態診断に役立つ病理組織所見を中心に述べる．
