要旨　同種造血幹細胞移植後に生じるGVHD（graft-versus-host disease）は，移植特有の合併症である．消化管病変は，主として急性GVHDでみられ，下痢は消化管GVHDのほぼ全例で起こる．消化管での好発部位は，上部では胃，下部では回腸末端～深部結腸とされ，その内視鏡像は，浮腫，発赤，びらん，潰瘍など非特異的であり，内視鏡像のみでの診断には限界がある．前治療の影響や感染症の合併は，より病態を複雑化するため，臨床所見に加え生検による病理診断が重要である．
