要旨　大腸鋸歯状病変は，近年の病理組織学および分子生物学的検討により，HP（hyperplastic polyp），SSA/P（sessile serrated adenoma/polyp），TSA（traditional serrated adenoma）に分類されることが提唱されている．SSA/P，TSAは癌化の可能性を有するが，pit pattern診断を含めた内視鏡的な特徴についての詳細な報告は少なく，今回，同病変に対する内視鏡診断の可能性とその治療方針を明らかとすることを目的に検討した．SSA/Pにおいて，HPとの鑑別には，10mm以上の病変径と病変表面の強い粘液付着という通常内視鏡所見を上回る有意なpit pattern所見は認めなかったが，NBI拡大観察での拡張・蛇行した血管VMV（varicose microvascular vessel）が有用であることが示唆された．TSAは，Is型でIII_H型pitを，Isp，Ip型でIV_H型pitを呈することが多く，その診断には，pit patternを用いることで容易であった．大腸鋸歯状病変の診断には，通常内視鏡所見に上記の拡大内視鏡所見を加えた総合的な診断のもと，内視鏡切除を決定することが現時点では妥当である．
