要旨　腸管Behçet病（BD）は非特異性炎症を呈し病理診断が難しいとされているが，しばしば鑑別が問題となるCrohn病（CD）とは大きく異なる組織学的特徴を有している．CDに比較した腸管BD病変の特徴は，肉眼的に境界明瞭な大型，円形～卵円形の打ち抜き様潰瘍が多いこと，割面のルーペ像は下掘れ傾向の強いフラスコ型潰瘍で潰瘍底は概して平坦であること，組織学的に潰瘍底の滲出壊死層が薄く，この層の好中球と肉芽組織層の形質細胞浸潤が軽いことである．CDにしばしばみられる腸壁を突き通すような線状・帯状の全層性炎症や非潰瘍部漿膜下のリンパ球集簇巣は腸管BDではまれである．腸管BDの潰瘍縁粘膜にもCDと同様の絨毛の萎縮，陰窩の配列異常，形質細胞浸潤が認められるが，CDよりもさらに限局性で軽度な変化にとどまる病変が多い．これらの特徴的差異に注目すれば，切除標本の大多数が診断可能である．単純性潰瘍は腸管BDとの異同が問題となっているが，肉眼的にも組織学的にも両者の間で明らかな違いは指摘されていない．
