要旨　免疫不全に対する単純性潰瘍様病変として，X染色体劣性遺伝形式をとる無汗性外胚葉形成不全免疫不全症候群が知られている．この疾患はNEMO遺伝子の異常であることが明らかとなり，消化管の潰瘍病変を合併しやすく，消化管病変ではNEMO腸症とも言われる．本症では，炎症性腸疾患に類似する回盲部潰瘍を形成しやすく，単純性潰瘍もしくはBehçet病との鑑別が必要である．歯牙形成不全，汗腺の欠如，寡毛，易感染性などの消化管管外症状を知っていれば鑑別診断は容易である．回盲部の潰瘍病変は数個のものから，回盲部から終末回腸に多発する潰瘍まで様々であるが，消化管病変に関する報告は少なく，NEMO遺伝子異常を確認した自験例を中心に解説する．
