要旨　Crohn病に対するインフリキシマブ（IFX）による治療には，維持投与中に効果が減弱する効果減弱例や効果が消失する二次無効例などのいわゆるIFX抵抗例が出現し，治療に難渋する症例がみられるようになった．これらIFX抵抗例は，投与法の工夫やアダリムマブへのスイッチにより，臨床的寛解，またはそれに準じた状態に持ち込めるが，小腸に広範な病変を有する症例や狭窄症例の多くは臨床的に寛解状態に持ち込むことが困難であり，画像所見でも粘膜治癒が得られていなかった．
