右側大腸腫瘍の臨床病理学的特徴

はじめに

　大腸癌を結腸癌と直腸癌に分けると，結腸癌，特に右側結腸癌の増加が著しい．年間登録でみると結腸癌が約15,000例，直腸癌が約10,000例である^1）．特に60歳以上の女性の右側結腸癌，なかでも50歳で左側大腸癌を手術し，70歳で再び右側の癌が発見される，いわゆるinterval cancerが増加している^2）3）．見逃しか高齢者発症であるかは別にして，低い平坦な病変，あるいは結節集簇，LST（laterally spreading tumor）^4）を背景にした癌が多いとされる．言い換えると，（1） K-ras遺伝子野生型でCpG islandメチル化形質（CpG island methylator phenotype ; CIMP）と共存するBRAF遺伝子異常〔後述する，SSA/P（sessile serrated adenoma/polyp）関連〕の癌が高齢者にみられること，（2） LSTの増加，である．LSTにはG（granular type）とNG（non-granular type）がある．LST-Gはras遺伝子変異を伴い，LST-NGはras遺伝子野生型が多いとされている^5）．LSTも右側大腸癌の臨床病理学的特徴を解析するうえで，重要な鍵となる病変であるが，原稿枚数が大幅に超過することになるので，定義も含めて主題論文に譲ることにする．（1） について，以下言及する．
