咽頭表在癌の特徴と鑑別診断―耳鼻咽喉科の立場から

要旨　近年開発された狭帯域光技術（NBI）などの内視鏡技術の向上により，これまで困難であった咽頭表在癌の発見が容易になった．咽頭表在癌をNBI観察した際の特徴は，1つは周囲との境界が明瞭な褐色調の領域として認められること，もう1つは病変内部に異型血管増生像が認識されることである．拡大機能をもたない咽喉頭用内視鏡でも，NBI観察によりこれら2点の特徴は十分視認可能であるため，咽頭表在癌の拾い上げ診断は可能である．咽頭表在癌との鑑別を要する病変として，low grade intraepitherial neoplasiaやglycogenic acanthosis，乳頭腫などがある．挿入経路の違いから，咽喉頭用内視鏡は上部消化管内視鏡と比べ軟口蓋裏面や舌根部の観察には優れているが，咽頭後壁の観察には工夫を要する．
