酢酸撒布（acetic acid instillation）

　消化管領域における酢酸撒布の報告は1998年Guelrudら^1）によるBarrett食道焼灼後の遺残円柱上皮島の識別に関するものが最初である．その後，拡大観察併用^2），胃癌での側方範囲診断^3），NBI（narrow band imaging）併用拡大観察^4），酢酸インジゴカルミン併用^5），十二指腸や大腸への応用など様々な進展をみせている．

　酢酸撒布法の原理は，pHの低下により粘膜細胞内のサイトケラチン重合化が促進され，粘膜表面が白色化することによる．円柱上皮では1.5％酢酸を撒布後，数秒で粘膜表面は白色化し表面構造が鮮明になる．その効果は可逆的であり，持続は数分程度と言われている．また，癌部は非癌部より白色変化が早期に消失するため，酢酸撒布後の経時的変化も胃癌の範囲診断に有用である^3）．
