demarcation line

　demarcation lineとは，一般的には，境界線，分画線，分界線を指し示す用語である．局在した粘膜病変の拡大内視鏡診断を行う際の指標に用いる場合，局在病変の背景粘膜と病変に存在する明瞭な境界線をdemarcation lineと称する（Fig. 1）^1）．demarcation lineの有無を判定するには，背景粘膜と病変の間に微小血管構築像〔MV（microvascular）pattern〕または表面微細構造〔MS（microsurface）pattern〕の急峻な変化（abrupt change）を認める場合を，demarcation lineありと判定する．

　demarcation lineは，VS classification systemによる癌・非癌の診断に有用な指標である^2）．それによると，demarcation lineの内側にirregular MV patternやirregular MS patternが存在する場合を癌と診断する（Fig. 2）．demarcation lineは，臨床的には胃炎と胃癌の鑑別診断^3）4）や早期胃癌の境界診断^5）に有用な所見であり，発赤した病変については，demarcation lineが存在しない場合は，非癌と診断することができ，陰性的中率（negative predictive value）が高い指標である．
