GIST（gastrointestinal stromal tumor）

　消化管間葉系腫瘍（gastrointestinal mesenchymal tumor；GIMT）の大部分を占める非上皮性腫瘍が消化管間質性腫瘍（gastrointestinal stromal tumor；GIST）である（Fig. 1）．GISTは胃に最も多く発生し，次いで小腸，大腸となり食道での発生はまれである．胃GISTのほうが小腸GISTより一般的に予後はよいと考えられている．

　過去，Stout^1）の報告以来，GIMTは大部分が筋層由来の腫瘍と考えられ，良性の平滑筋腫と悪性の平滑筋肉腫，核周囲に空胞や淡明領域をもつ上皮様腫瘍の平滑筋芽腫に分類されていた．しかしMazurら^2）が電子顕微鏡や免疫染色で詳細に検討し，これらの腫瘍は平滑筋細胞の形質を持つものが少なく，Schwann細胞への分化を示す腫瘍も存在することを示した．
