大腸憩室出血の画像診断と治療―特に造影CT検査を併用した内視鏡治療の有用性について

要旨　大腸憩室出血に対する診断の問題点として，憩室が多発する場合が多く，出血部位の同定がしばしば困難である点が挙げられる．筆者らの検討では，できる限り早期に造影CT検査を施行して，造影剤の血管外漏出像を描出し，下部消化管内視鏡検査（CS）前に憩室出血部位を推定することで，効率的に出血点を同定することが可能と考えられる．大腸憩室出血に対する内視鏡治療として，本邦ではクリップ法が選択されることが多いが，クリップ法は3割程度に再出血を認め，特に活動性出血例では入院中の早期再出血を高率に認めるため，より確実な止血方法の工夫が必要と思われる．EBL（endoscopic band ligation）は，活動性出血例においても高い止血効果が得られており，大腸憩室出血に対する非常に有効な治療法であると考えられる．
