要旨　家族性大腸腺腫症（familial adenomatous polyposis ; FAP）およびLynch症候群は，共に常染色体優性遺伝の疾患であるが，その臨床像は大きく異なる．FAPのうち，特に密生型は大腸全体にポリープが無数に多発し，標準治療として大腸全摘術が推奨され，大腸外病変として十二指腸癌，胃癌，デスモイドの発生率が高い．しかし，適切な外科手術および内視鏡によるサーベイランスにより長期生存が可能となってきた．一方，Lynch症候群は，内視鏡所見や手術所見からのみでは同定が難しく，マイクロサテライト不安定検査がスクリーニング方法として保険収載されているが，普及は十分ではない．両疾患共に小腸病変に関する本邦からのエビデンスは少なく，今後の症例集積が急がれる．
