好酸球性消化管疾患の考え方

好酸球性消化管疾患（eosinophilic gastrointestinal disorders ; EGID）は，消化管に好酸球が浸潤する慢性炎症性アレルギー疾患の総称とされ，最近では全消化管の罹患部位にかかわらず，好酸球浸潤が基本となる病態にこの概念が用いられるようになった1）．ただし，EGIDの同義語としては，EGIDの他にeosinophilic gastrointestinal diseases（Furutaら，2008）2），eosinophil-associated gastrointestinal disorders（DeBrosseら，2008）3），primary eosinophilic disorders of the gastrointestinal tract（Yanら，2009）4），eosinophilic digestive diseases（Shiffletら，2009）5），eosinophilic diseases of the gastrointestinal tract（Lucendoら，2012）6）などの記載が最新の英文論文にある．
また，この訳語としては，“好酸球性胃腸疾患”7），“好酸球性消化管障害”8），“好酸球増加症候群”9）などがあり，必ずしも本邦で統一されておらず，今後は呼称の統一が望まれている．その先駆けとして，厚労省研究班 : 消化管を主座とする好酸球性炎症症候群（正式名称 : 好酸球性消化管疾患）の診断治療法開発，疫学，病態解明に関する研究班（研究代表者 野村伊知郎）9）では，本特集号で用いる「好酸球性消化管疾患」をEGIDの正式の呼称とすることを決定した．その理由としては，EGIDの概念には，病態が異なる疾患が多数含まれるとの認識による．EGIDの病態解明が進めば，欧米での呼称と概念も統一されるものと推測する．
