要旨　大腸ESDは，胃などに比べると難易度が高いと言われている．その理由として，大腸は長い管腔臓器であり内視鏡の操作性が不安定になりやすいことや，ひだや彎曲の存在により，病変とESDナイフが垂直に対峙しやすいことなどが考えられる．ダブルバルーン内視鏡は，小腸の観察・治療を目的として開発されたが，そのオーバーチューブバルーンで腸管のたわみやループを抑制することで，内視鏡の操作性を安定させることが可能である．筆者らは2004年9月から2012年3月の期間に行った大腸ESD 632病変中，通常内視鏡ではESD困難と考えられた58病変にダブルバルーン内視鏡を用いたESDを行い，その有用性を確認した．また，垂直に対峙する病変に対してはSAFEKnifeVが有用であると考えられた．
