炎症性腸疾患における妊娠と出産

炎症性腸疾患（IBD）合併妊娠の管理は，海外のエビデンスをもとに，疾患コントロールと治療薬の安全性のバランスを考慮し決定する．IBDであるだけでは不妊率は増加せず，寛解期に妊娠したIBDの再燃率は，非妊娠者と同等である．IBD治療薬の多くは妊娠への悪影響を立証されておらず，IBD合併妊娠の最大の危険因子は疾患活動性とするのが，近年の海外における主流の考え方である．IBD患者では，早産，低出生体重，奇形など合併症の危険がわずかに増えるが，寛解期では増加せず，活動期で増加することから，母親の疾患活動性を良好にコントロールすることが，妊娠転帰を改善し望ましいと考えられる．
