消化管内分泌細胞腫瘍の概念と分類―その歴史的変遷

初期の概念

　内分泌細胞性腫瘍は，1888年にLubarsch^1）によって，消化管の癌に類した良性腫瘍として報告され，その後Oberndorfer^2）により本腫瘍は組織学的には異型の低い，特徴的な細胞配列を示す腫瘍で，発育は緩徐で転移の少ない予後良好の腫瘍として，カルチノイド腫瘍と命名した．その後Pearsonら^3）が本腫瘍の中には転移を来すものが少なからずあると報告し（42例中16例，38％），それ以来悪性腫瘍として認識されるようになった．

　本腫瘍は銀親和性を示すことから，内分泌細胞性格を有しKultzchutzky細胞由来と想定され^4），さらにセロトニンないしはヒスタミンを産生し，カルチノイド症候群を呈することも明らかにされてきた^5）．また電子顕微鏡で腫瘍細胞内に神経分泌顆粒（neurosecretory granule）を有していることも報告された^6）．
