大腸ポリープ雑考

大腸ポリープの分類

　大腸ポリープが病理学的に種々の疾患カテゴリーで構成されていることは消化器の専門医であれば誰でも知っている．そしてこれらに共通するのが，大腸内腔に向かって限局性に隆起する病変，という肉眼的形態であることもコンセンサスがとれているものと思われる．しかしながら，筆者がこの序説を書くに当たって，いくつかの書物を調べてみたが，“取扱い規約”にも“ガイドライン”にも“WHOのテキスト”にも独立して大腸ポリープの分類は取り上げられていない．

　一方，世界的なテキストである「Morson and Dowson's Gastrointestinal Pathology」^1）には大腸ポリープの分類が提示されていた．その分類に若干の改変を加えた分類をTable 1に挙げた．この分類をみると，誠に多くの病変が大腸ポリープを構成していることがわかる．もちろんこの分類も完成されたものではなく，今後もいくつもの病変がこの中に加わることになると思われる．
