要旨　囊胞状に拡張した腺管や炎症性間質を特徴とする弧発性の非腫瘍性大腸ポリープの診断は，内視鏡的にも病理組織学的にも混乱している部分があると思われる．本学附属病院および関連施設で内視鏡切除により得られた弧発性の炎症性間質を認める大腸ポリープ77例〔IMGP（inflammatory myoglandular polyp）36例，弧発性若年性ポリープ10例，“いわゆる弧発性炎症性ポリープ”31例〕を対象とした．IMGPは，病理組織学的には過形成性腺管と囊胞状に拡張する腺管，炎症性間質，ポリープ基部の平滑筋の挙手状増生が特徴的な所見であった．一方若年性ポリープ，“いわゆる弧発性炎症性ポリープ”には，上記の所見のうち，過形成性腺管，ポリープ基部の平滑筋の挙手状増生はほとんどみられなかった．粘液形質は，いずれのポリープにも胃型形質を含む例が多く，腺管単位でp53過剰発現の頻度には差異は指摘できなかった．IMGPは，臨床病理学的のみならず病理組織学的にも他のポリープとは異なった特徴を有しているポリープと考えられた．
