食道SM癌に対する治療成績―CRTの立場から

要旨　本邦における切除可能食道扁平上皮癌に対する標準治療は，食道切除術である．一方で，化学放射線療法でも食道切除術と比べて遜色ない治療成績が得られるようになってきた．JCOG9708の結果から，Stage I食道癌に対する根治的化学放射線療法の高い有効性が示唆され，標準治療である食道切除術と根治的化学放射線療法を比較する臨床試験が現在進行中である．また，近年内視鏡治療の開発も進み，内視鏡治療の結果非治癒切除と診断される事例も増え，こうした対象への追加治療としての化学放射線療法の有用性についても検討がなされている．治療後のQOLを保つ侵襲の少ない治療法としての化学放射線療法の確立，そしてJCOG0502/0508の結果に期待が寄せられている．
