食道亜全周・全周ESD後の狭窄予防―ステロイド内服の立場から

文献概要

要旨　食道ESDは2008年に保険収載となり，難易度がやや高いものの，その低侵襲性・高い根治性により全国的に広く普及してきている．一方で，3/4周以上の広範囲剝離症例では術後狭窄が高頻度に起こり，頻回の拡張術が必要となるため，患者や医療経済上の負担が大きいことが問題となっており，その克服が食道ESD普及の最大の課題となっている．このような広範囲剝離症例に対し，筆者らはステロイド経口投与もしくは局注療法の併用により拡張術回数・治療期間を有意に低減可能であることを報告してきた．そこで今回，ステロイド経口投与・局注療法の有用性とその限界，また，安全な局注のために筆者らが開発した局注針Nタイプに関して概説する．
