要旨　潰瘍性大腸炎の長期経過例では，慢性持続炎症を背景とした大腸癌の合併リスクが高いことから，大腸内視鏡検査によるサーベイランスが推奨されている．しかし，その病変は，通常の散発性大腸癌と比較して内視鏡的に同定困難であるとされてきた．このため，これまでは盲目的な生検で大腸癌や前癌病変であるdysplasiaを確率的にとらえようとするランダム生検が行われてきたが，侵襲の大きさと同定率の低さが費用対効果に見合わないことが問題であった．近年，内視鏡の解像度の向上に加え，色素内視鏡や拡大観察などの応用で以前より病変の視認性が向上してきていることを背景に，狙撃生検の有用性が示唆されるようになってきた．現在本邦で行われているランダム生検と狙撃生検の無作為ランダム化比較試験の結果が待たれるところである．
