はじめに

　リンパ腫様胃症（lymphomatoid gastropathy ; LyGa）は，胃に生じる原因不明のNK細胞増殖症で，消化管悪性腫瘍，特にリンパ腫と間違えられやすい良性病変である．組織学的には，悪性度の高いextranodal NK/T-cell lymphomaとの鑑別が問題となる．しかしながら，LyGaは自然消退し，予後は良い．したがって，LyGaの概念を知らなければ，容易に過剰診断・過剰治療につながる．

　筆者ら^1）は，最初の10例を2010年にLyGaとして報告したのち，これまで主に病理医，血液内科医を対象として概念の周知を図ってきた．しかし，実際に内視鏡検査を行う医師に対しては情報提供が十分と言えない状況であった．LyGaは組織像のみならず，その内視鏡像も特徴的であるので，内視鏡医の間で周知されれば発見頻度は高くなり，過剰診断防止にもつながる．本稿ではLyGaの内視鏡像，臨床病理学的特徴を中心に説明する．
