消化管病変を認めたgiant cell arteritisの1例

要旨●患者は88歳，男性．主訴は下腹部痛．腹部造影CTにて腸閉塞と診断され入院加療となった．腸閉塞改善後の小腸X線造影検査では終末回腸に腸間膜付着対側を中心とする偏側性変形，偽憩室様変化を，小腸内視鏡検査では縦走から一部輪走する浅い潰瘍，狭小化を認めた．大腸内視鏡検査では盲腸には不整形の浅い潰瘍，回盲弁と上行結腸には輪状潰瘍を認めた．生検による病理組織学的検査，その他の培養，血液検査でも確定診断には至らなかったが，経口摂取後も腸閉塞症状を繰り返していたため回盲部切除術を施行した．病理組織学的所見は粘膜下層に存在する小型から中型の動脈周囲に，多核巨細胞を含む炎症細胞浸潤と肉芽腫様病変を認め，内弾性板の変性や断裂の所見も認められた．頭部症状がなく側頭動脈生検は施行していないが，年齢，赤沈値の亢進，巨細胞性動脈炎（GCA）に特徴的な病理組織像から，腸管に限局したnon-cranial GCAと診断した．GCAの消化管病変の報告は極めてまれであり，その特徴を明らかにするにはさらなる症例の蓄積に基づく検討を要する．
