大腸鋸歯状病変の内視鏡診断

要旨●大腸鋸歯状病変は，過形成性ポリープ（HP），SSA/P，TSA，SSA/P with cytological dysplasiaの4者に大別されるが，本稿ではHP，SSA/P，TSAの内視鏡的特徴について概説した．HPとSSA/Pは一見類似した病変であるが，SSA/PはHPに比べて腫瘍径が大きく，局在が右側>左側結腸であり，粘液付着，開II型pit patternの割合が高かった．TSAは局在が左側>右側結腸であり，ほとんどが隆起型で，pit patternはIII_H型またはIV_H型の割合が高く，TSAの診断はpit patternを用いることで容易であった．部分的に小隆起を認める場合や，雲状発育を呈したSSA/Pの一部が盛り上がっている場合は，同部でSM浸潤を来している可能性があり，拡大内視鏡観察を含めた詳細な診断が必要である．
