大腸鋸歯状病変の経過と取り扱い─癌併存病変の内視鏡診断

要旨●近年，serrated pathwayという鋸歯状構造を有する大腸病変からの発癌経路が提唱されたことから大腸鋸歯状病変が注目されているが，その病変の取り扱いについては，さまざまな論議が行われているところである．今回筆者らは，そのような病変についていかに取り扱うべきかを明らかにするため，過形成病変，広基性鋸歯状病変，古典的鋸歯状腺腫について，その内視鏡所見と病理組織学的所見を検討した．その結果，通常内視鏡において発赤，陥凹，二段隆起の所見が存在し，NBI拡大観察においてCP Type III所見を認めたり，pit pattern拡大観察においてV型pitの所見が観察されたりした際には癌の併存を強く疑い，積極的に病変を摘除することが望ましいと考えられた．
