早期大腸癌の深達度診断─NBI拡大観察の有用性と限界

要旨●NBI拡大診断（CP Type IIIB）のT1b癌に対する診断能は，pit pattern診断（invasive pattern）と比べて，感度は同等（84.8％ vs. 85.6％）であるが特異度が低い（88.7％ vs. 99.4％）．これは臨床的観点からすると，NBI拡大診断（CP Type IIIB）はT1b癌と診断したときにoversurgeryとなりやすいことを意味しており，現状では，より特異度の高いpit pattern診断が深達度診断のgold standardと言えるだろう．
