要旨●食道胃接合部癌は，食道と胃の境界領域に位置するという解剖学的特殊性のために食道癌として扱うべきか胃癌として扱うべきか議論があり，術式選択の基準とすべきデータがなかった．日本胃癌学会・日本食道学会合同で行われた全国調査により，暫定的基準としてのリンパ節郭清アルゴリズムが策定され，「胃癌治療ガイドライン 2014年5月改訂第4版」に収載された．No. 4sb，4d，5，6といった幽門側胃の壁在リンパ節は，予防的リンパ節郭清の効果が限定的と考えられ，郭清推奨範囲に含まれていない．腫瘍径4cm以下の場合には胃全摘術は必ずしも必要がないという暫定的結論であり，今後の術式選択に一定の方向性が出てくるものと考えられる．
