要旨●本邦においてプロトンポンプ阻害薬（PPI）使用量は確実に増加しており，特に高齢者においてその傾向は顕著である．近年，胃炎の内視鏡診断の重要性が周知されるようになり，PPI使用に付随する内視鏡所見を認識することも臨床的に重要になっている．PPI投与により壁細胞は変性し，組織学的にPCP（parietal cell protrusion）と呼ばれる形態を示す．H. pylori未感染者において，胃底腺ポリープはPPI服用で増加することが報告されており，特に水腫様の内視鏡像が特徴的とされる．一方，過形成性ポリープはH. pylori陽性者にみられるが，PPIによる二次的高ガストリン血症によりポリープが増大する．体部にみられる多発白色扁平隆起もPPI投与例で多くみられ，組織学的には腺窩上皮の過形成性変化が認められる．
