要旨●潰瘍性大腸炎の長期経過例では癌発生率が高く，内視鏡検査による癌サーベイランスが必要である．炎症のある粘膜に発生し，境界不明瞭で平坦な病変が多いため，早期発見が難しいという問題点がある．サーベイランスの効率を高めるため，高危険群を絞り込み，IEEを併用した拡大内視鏡検査による精密な内視鏡診断を行うことが重要である．自験例の粘膜内腫瘍の検討では40％近くが平坦型病変であり，いずれも発赤領域として認識されている．pit patternはIV_V型の頻度が高いが，平坦型病変に限ると管状pitが約70％を占めている．初期病変である平坦型病変の発見には，わずかな隆起や領域のある発赤に注意し，積極的な拡大観察を行うことが重要である．
