除菌後発見胃癌の1例

要旨●患者は70歳代，女性．13年前にH. pylori除菌治療を受けていた．検診目的に施行された上部消化管内視鏡検査にて異常を指摘されたため，当院へ紹介され受診となった．通常内視鏡にて胃体中部前壁に3mm大の褪色調の扁平隆起を認めた．NBI拡大観察では，腫瘍の境界は明瞭で内部の顆粒状構造と微小血管に不整像がみられたため，早期胃癌と診断した．ESDを施行し，病理組織学的診断では，3×3mm，tub1，pT1a，ly（−），v（−），HM0，VM0であった．免疫組織化学染色像で腫瘍はMUC5AC陽性細胞が主体であり，腺窩上皮型の低異型度高分化管状腺癌と診断された．本症例は除菌後発見胃癌の初期像の1例と考えられた．
