【大腸】鋸歯状病変

はじめに
　大腸鋸歯状病変は，現在の過形成性ポリープ（hyperplastic polyp ; HP）を1962年Morson^1）によりmetaplastic polypという用語で紹介されたのが最初で，鋸歯状構造を示す非腫瘍性の数mm大の直腸に好発する病変で，癌化の危険性はないとされていた^1）〜3）．そして，1990年にLongacreとFenoglio-Preiser^4）により鋸歯状構造を示すが，腫瘍性病変である鋸歯状腺腫（serrated adenoma ; SA）という概念が提唱された．SAは左側大腸に好発し，癌化の危険性は管状腺腫と同等と考えられていた^4）5）．さらに，1996年にTorlakovicとSnover^6）は，鋸歯状ポリープのうちHPやSAとは区別すべき腫瘍性のものがあると主張し，SSA（sessile serrated adenoma）という用語を用いた．その後，SSAは組織像からは腫瘍性とは判定できないことからSSA/P（sessile serrated adenoma/polyp）^＊1という名称が一般的となっている^7）．SSA/Pは高率にBRAF変異を有しマイクロサテライト不安定性（microsatellite instability ; MSI）^＊2を示す大腸癌の前駆病変として注目されている．なお，英国ではSSL（sessile serrated lesion）という名称を用いている^8）．
　現在のWHO分類において大腸鋸歯状病変^＊3は，HP，TSA（traditional SA），SSA/Pに分類され，さらにSSA/Pに粘膜内癌を含む明らかな腫瘍を併存するものはSSA/P with cytological dysplasiaという用語で表現される^7）．本邦の「大腸癌取扱い規約 第8版」^9）もWHO分類に準じた分類を採用している．
　また，Jassら^10）は形態および粘液組織化学的に鋸歯状腺腫と類似性のある癌に対して“serrated carcinoma”という言葉を初めて用いたが，Mäkinen^11）のグループが鋸歯状構造を認める癌を“serrated adenocarcinoma”という概念で包括し，MSIが高率に認められ，分子生物学的因子（EphB2，PTCH，HIF1α）が通常の大腸癌と異なることより，大腸癌の一亜型として分類する意義があると報告している．WHO分類では大腸癌の組織亜型としてserrated adenocarcinoma（鋸歯状腺癌）という名称で記載されている^12）．
　本稿では，鋸歯状腺癌も含めたこれら大腸鋸歯状病変の病理組織学的特徴とそれらの鑑別のポイントについて解説する．
