要旨●高周波通電を行わないcold polypectomyが，小型のポリープ摘除法として普及しつつある．CFPは，鉗子を用いたcold polypectomyであり，カップの大きなJumbo鉗子を用いることで，5mmまでの微小腺腫が簡便，迅速，安全に切除可能で，切除標本の回収も100%である．これまで後出血や穿孔の偶発症の危険性はほとんど報告されていないが，高周波通電によるburning effectがない分の局所再発を抑制するには，Jumbo鉗子の使用で一括切除率を担保し，病変摘除直後のwater-jet機能を用いたタンポナーデ形成とNBIなどの画像強調内視鏡を用いた切除直後の遺残の確認が必須と考える．これにより，今後，抗血栓療法中患者を含め，CFPが広く適応されることを期待する．
