咽頭brownish areaの鑑別診断と取扱い

要旨●上部消化管検査時に，咽頭領域を観察することの重要性が広く認知されるようになり，内視鏡医がこれらの領域の病変を発見する機会が増加している．内視鏡室での観察時にヨード撒布が困難な咽頭・喉頭領域では，NBIをはじめとする画像強調機能を有する拡大内視鏡が病変の拾い上げ，質的診断に有用なモダリティーである．咽頭領域のbrownish areaの鑑別診断は，領域性の有無と血管形態の変化を中心に，咽頭癌，リンパ濾胞，乳頭腫，メラノーシス，炎症，化学放射線後の変化を鑑別していく．咽頭癌であれば治療適応となるが，その他の病変は基本的に経過観察が推奨される．無治療で経過観察したbrownish areaについては，組織学的に癌と診断され10mm前後のサイズのものは増大すると報告されているが，5mm以下の小さなものはあまり増大しないようである．
