要旨●コンパニオン診断は分子標的治療薬の効果が期待できる患者を選定するための重要な検査である．免疫組織化学染色を用いた消化器癌のコンパニオン診断はトラスツズマブの投与対象であるHER2過剰発現胃癌に行われている．胃癌特有の問題としてHER2過剰発現の腫瘍内不均一があり，1切片で実施されたHER2判定がその患者の腫瘍全体を反映しているとは限らないことを考慮し結果を解釈する必要がある．コンパニオン診断における免疫組織化学染色は，組織固定〜包埋過程，染色過程，判定過程のすべてにおいて，推奨された適切な方法で実施されることで初めてその結果が担保される．今後コンパニオン診断がさらに増えていくことが予想され，検体の質の担保を継続してはかっていくことが重要である．
