特殊組織型食道癌の拡大内視鏡診断

要旨●食道表在癌における特殊組織型は，その頻度が低いことから，拡大内視鏡での検討がほとんど成されていない．自験例での検討では，粘表皮癌は，拡大内視鏡での血管像で明瞭なものはないが，粘液成分を表面に有し，癌の導管進展を伴うことがあることが認識された．腺扁平上皮癌は，表層まで腫瘍が露出するとreticular patternを呈し，深部にのみ存在する場合は，通常の扁平上皮癌と同じである．小細胞癌は褪色調の粘膜下腫瘍様形態を呈し，reticular patternが認められる．類基底細胞癌は，赤みが強く，ループ形成のない拡張した血管が密に増生していることが特徴として挙げられた．拡大内視鏡のみでの診断は困難であり，肉眼形態や付帯する所見を加味して診断することが必要である．
