拡大内視鏡が早期胃癌の診断をどう変えたか─胃拡大内視鏡は，スクリーニング内視鏡におけるoptical biopsyになり得るか?

要旨●背景と目的：胃拡大内視鏡による早期胃癌診断の有用性について多数の報告がある．しかしながら，このモダリティーが実際の臨床（clinical practice）において何を変えたかは，明らかでない．そこで，ルーチン検査において，小さくまたは平坦な癌を診断するために採取されていた生検の数を減らした否かを求めた．対象と方法：1人の内視鏡医が，上部消化管拡大内視鏡を用いルーチン検査を行った連続した患者を対象に“早期胃癌1病変を診断するために必要であった生検の個数”を求めた．結果：上部消化管拡大内視鏡を用いルーチン検査を806例の患者に対し施行し，早期胃癌1病変を発見するために必要であった生検の数は，3.8個（生検数：34個/早期胃癌数：9病変）であった．考察：胃癌内視鏡検診で早期胃癌1病変を発見するために採取された生検の数は少なくとも34.1個であり，この数値と比較すると，拡大内視鏡をスクリーニング検査に用いれば，早期胃癌を診断するために採取されている生検数を減らせるという点でclinical practiceを変えていることが示唆された．結語：胃拡大内視鏡は早期胃癌診断に必要な生検数を減らすという点でclinical practiceを変えたoptical biopsyとして有用かつ非侵襲的な診断法である．
