要旨●潰瘍性大腸炎の内視鏡検査では白色光観察のほか，画像強調内視鏡（IEE）を組み合わせることで疾患の診断や，より詳細な重症度・治療効果の評価が行われてきている．IEEには消化管組織から出る微弱な自家蛍光を画像化し病理組織学的な炎症強度を数値として評価する客観性の高い自家蛍光内視鏡（AFI）や，粘膜表層の微細構造変化から炎症を評価する拡大内視鏡などがある．これらのIEEによる炎症の評価は病理組織学的活動性と相関するため，重症度や粘膜治癒の評価，再燃の予測が可能となり，最適な治療選択と長期の寛解維持へとつながることが期待される．
