要旨●潰瘍性大腸炎は内視鏡的寛解が長期予後の予測に有用であることが知られているが，近年は組織学的寛解が重要視されるようになってきた．組織学的活動性評価には生検が必要であるが，生検は局所的な評価である．またわずかであるが出血や穿孔といった偶発症の可能性もある．近年新しいデバイスとして超拡大内視鏡（ECS）が登場した．ECSは最大450倍まで拡大し，腺窩を観察することが可能になった．筆者らはその拡大所見をECS scoreで数値化し，組織学的活動性が相関することを示し，さらに内視鏡的寛解期においても組織学的活動性を評価することが可能である．ECSは再燃リスクの新たなモニタリングツールとなりうる可能性がある．
