要旨●Crohn病において小腸病変評価は治療方針を決めるうえで重要である.バルーン内視鏡の普及により小腸病変を詳細に評価できるようになった.定量的な内視鏡評価は重要であるが,現在小腸を定量的に評価可能な内視鏡スコアは存在しない.本稿では当施設での内視鏡評価方法および内視鏡スコアについて紹介する.またバルーン内視鏡検査は臨床的に非常に有用である一方,検査可能な術者や施設が限られる点などを考慮すると,内視鏡の代用となりうる検査についても重要な課題であり,当施設で行っているバルーン内視鏡とMRI検査との直接比較についても報告する.今後,バルーン内視鏡検査の利点,欠点をよく検討し,その他の検査も含めた診療のストラテジーを構築する必要がある.
