疾患の頻度と概念の変遷
　近年，本邦で徐々に増加している疾患が好酸球性食道炎（eosinophilic esophagitis ; EoE）である．筆者が初めてこの疾患を知ったのは，2005年5月にシカゴで開催された第106回米国消化器病週間である．本疾患は，アレルギー，特に食物アレルギーが関与する疾患であり，成人だけではなく小児にも認められ，狭窄のため嚥下障害を来す疾患であることに驚かされた．
　日本に帰り，早速，PubMedでEoEをキーワードに検索をかけると，既に1960年代から海外では報告がなされていたが，当初は好酸球性胃腸炎（eosinophilic gastroenteritis ; EGE）の食道病変としての報告であった^1）〜5）．EoEとしての最初の報告は，1978年のLandresら^6）の心窩部痛を主訴として受診した44歳，男性例で，食道内圧検査でvigorous achalasiaと診断された症例に合併したものとされている．EGEの一病変として好酸球浸潤が食道に起こることは，Landresら^6）の報告の1年前，1977年にDobbinsら^2）が報告している．
