─早期胃癌の治療と予後─早期胃癌に対する内視鏡治療の現状

要旨●早期胃癌に対するESDは技術的には既に成熟したと考えられる．適応拡大についても多施設前向き臨床試験（JCOG0607）の結果を受けて，ようやく分化型適応拡大病変（2cm超のUL0と3cm以下のUL1）がESD絶対適応として認められることとなった．未分化型についても間もなく適応拡大が期待されており（JCOG1009/1010），標準治療としての適応についてはほぼ完成すると考えられる．一方で，今後は高齢化する社会および胃癌患者に対応すべく，その適応や治癒基準についても検討していく必要がある．
