特殊な食道腫瘍の臨床病理学的特徴

要旨●扁平上皮癌，腺癌を除く食道腫瘍の中の，類基底細胞癌，癌肉腫，腺扁平上皮癌，神経内分泌細胞癌，粘表皮癌を取り上げ解説した．これらは食道悪性腫瘍全体の10%にも満たない比較的まれな腫瘍である．これら特殊な食道腫瘍は病理組織学的多様性に富む腫瘍であり，扁平上皮癌や腺癌の併存率も高い．それゆえ生検を施行しても治療前に最終診断に至らないことがある．しかし，それぞれの腫瘍は肉眼的に粘膜下腫瘍様形態を示すなど，詳細にみれば臨床病理組織像が通常型扁平上皮癌と異なるため，切除検体では慎重に鑑別診断を進め，適正な治療に導くことが重要である．
