疾患の概念と最近の動向
　非結核性抗酸菌（nontuberculosis mycobacteria；NTM）は，結核菌群と培養不能ならい菌を除いた抗酸菌の総称である．以前は非定型抗酸菌と称されていた．自然環境のみならず水道やその配管，風呂場などの生活環境にも広く分布している．かつては弱毒菌と考えられていたが，感染臓器で最も多い肺に関しては，2014年に国立感染症研究所などが行った調査で肺NTM症の罹患数は14.7/10万人であり，肺NTM症の急激な増加と，初めて結核の患者数を上回ったことが明らかとなった^1）．現在，NTMは約150種類が同定されているが，ヒトに病原性を有するのはそのうち50種類程度であり，そのほとんどがMycobacterium aviumとMycobacterium intracellulareから成るMAC（Mycobacterium avium complex）である．そのため本稿では消化管MAC症を中心に記載する．
　MACは周りの環境から直接感染すると考えられているが，主な感染経路は経気道感染，経腸感染である．結核菌群と異なり，ヒト—ヒト感染は起こらないため，感染者の隔離は不要である．感染者の免疫機能が正常な場合は，経気道感染により肺MAC症を引き起こすが，免疫機能が低下している場合や小児では，MACは腸管粘膜より直接浸潤しマクロファージに取り込まれ，消化管MAC症を引き起こすと考えられている．
