手つなぎ・横這い型胃癌の病理学的新知見

はじめに
　手つなぎ型胃癌とは，腸上皮化生腺管に類似した細胞異型の乏しい腺管から構成され，あたかも腫瘍腺管同士が手をつないでいるような“手つなぎ”型吻合をはじめ，特徴的な構造異型を示す低異型度分化型腺癌である．特徴的な腫瘍腺管構築をアルファベットの形にたとえ，“WHYX lesion”と称されることもある．粘膜内，特に増殖帯の高さを中心に，水平方向に這うように進展する例が少なからず経験されることから“横這い型胃癌”とも呼ばれ，ほぼ同義語として使用される^1）．
　腸上皮化生と類似した低異型度癌のため，生検診断が難しい例がまれではない^2）．病理組織学的特徴を反映して内視鏡検査での範囲診断が困難な例も経験される^3）．このような胃癌が存在することを認識し，その特徴を知っておくことが肝要である．　近年の胃癌ゲノム研究の進歩により，手つなぎ型胃癌の遺伝子異常についてもその一端が明らかになってきた．この癌の病態解明や治療戦略を検討するうえで重要な知見と考えられ，後半で紹介する．
