要旨●代表的な小腸悪性腫瘍として，上皮性腫瘍の癌と神経内分泌腫瘍，および非上皮性腫瘍の悪性リンパ腫，間葉系腫瘍，転移性腫瘍が挙げられる．原発性小腸癌は潰瘍性腫瘤を呈し，内腔の狭小化と口側腸管の拡張が認められる．これに対して神経内分泌腫瘍は表面のびらん，潰瘍に加えて粘膜表面の増生した血管を伴う粘膜下腫瘍（SMT）様の像が特徴的であり，神経内分泌細胞癌では深い潰瘍性病変を伴うようになる．一方，悪性リンパ腫の肉眼像は多彩であるが，隆起型，潰瘍型，MLP型，びまん型，混合型に細分類することで組織型との対比が可能となる．間葉系腫瘍にはgastrointestinal stromal tumor，平滑筋肉腫，脈管系悪性腫瘍，脂肪肉腫などが含まれ，基本的にはSMT像を示し，しばしば表面に潰瘍や血管増生を伴う．転移性腫瘍のうち，血行性転移ではSMT様に立ち上がり伸展性良好な潰瘍性病変を呈し，直接浸潤では腸管の長軸方向に粘膜ひだの収束像がみられる．前述の特徴を熟知することで小腸悪性腫瘍の診断能が向上すると考える．
