はじめに
　早期胃癌に対する内視鏡的粘膜下層剝離術（endoscopic submucosal dissection；ESD）は，その低侵襲性により耐術能の低い高齢者に対しても適応しやすい手技として認識されている．しかし，主な偶発症として挙げられる後出血は抗血栓薬の服用が危険因子の一つとされ，抗血栓薬の服用頻度の高い高齢者においては特に留意すべきものである．一方で，抗血栓薬休薬による血栓塞栓症のリスクも看過できず，安易な休薬は極めて重篤な合併症の引き金になりかねない．したがって，抗血栓薬服用者に対する胃ESDにおいては，抗血栓薬を継続することで可能な限り血栓塞栓症のリスクを回避しつつ，後出血のリスクを極力低減することが理想となる．
