食道：扁平上皮癌

概念・定義
　扁平上皮癌とは，正常重層扁平上皮としての特徴である角化あるいは細胞間橋がみられる上皮性悪性腫瘍である．角化とは，上皮層最深部に位置する基底細胞が，表面へ向かって傍基底細胞，多辺形の有棘細胞と次第に核細胞質比を下げ扁平化し，最終的には核を失い剝離していく一連の変化を指す（Fig.1）．食道癌の発生母地である食道粘膜は，腺上皮に置換されたBarrett食道以外は重層扁平上皮に覆われるため，本邦においては食道癌の90%以上が扁平上皮癌である．
