●「考える内視鏡診断」のポイント
･食道扁平上皮癌の内視鏡診断では，治療方針選択につながる深達度診断が最も重要である．
･深達度診断において，まずは通常観察で食道を十分伸展させ肉眼型を決定する．
･次に送気を調整しつつ，病変の硬さや緊満感などを評価する．ヨード染色による畳目模様も有用である．
･NBI拡大観察では，JES分類を用いて血管評価を行う．
･通常観察とNBI拡大観察において，深達度診断に乖離を認めた場合は，積極的にEUSを行い最終決定する．
